Welcome to Riverdale Middle School!  Please be prepared to follow CDC guidelines wearing a mask is optional. To limit the number of people in the building, only the person enrolling the student will be allowed.  The following documents are needed for complete enrollment: 

·         Birth Certificate

·         Guardianship or custody papers if not the birth parent

·         Student Social Security Card

·         Georgia Immunization Certificate Form (Form 3231)

·         Georgia Eye, Ear and Dental Certificate (Form 3300)

·        Withdrawal records from previous school including last grade placement, report card, transcript, IEP, 504, gifted verification, discipline history

·         Parent ID

·         Current verification of residency (two proofs in the parent/guardian’s name)- lease or mortgage statement, utility bill (gas, electric, water)
